.07 Reporting.

A. A facility shall:

(1) Keep a record of each student's:

(a) Application;

(b) Current address and telephone number;

(c) Date of enrollment;

(d) Attendance; and

(e) Number of hours completed; and

(2) Make the record available to the Board at the Board's request.

B. At the end of each course, the program shall submit a report to the Board on a form required by the Board.

C. A program shall submit to the Board for approval any changes made to an approved program.
