31.10.04.01.htm 31.10.04.01. 01 Scope.. A. This chapter applies to all insurers writing health insurance in this State.. B. This chapter does not apply to health benefit plans issued under Insurance Article, Title 15, Subtitles 12, 13, or 14, Annotated Code of Maryland.
