Upon receipt of notification of a major adverse reaction to a pertussis vaccine, the local health officer shall:1) Notify and forward the morbidity report card to the Department within 24 hours; and. 2) Assure that the necessary information to fill out a "Report of Adverse Event Following Immunization" is obtained.B.
