10.44.20.01. 01 General.. A. A person shall pay a fee as set forth in Regulation .02 of this chapter by check or money order.. B. The Board shall provide a copy of this chapter:. 1) Along with any application form or renewal form; and. 2) To any other person on request.. C. The Board shall refund the initial licensure or renewal fee, less a $50 processing fee, to a dentist or a dental hygienist who does not qualify for initial licensure or renewal.
