10.25.05.03.htm 10.25.05.03. 03 Extension of Time.. A. A carrier may request an extension of time for submission of the survey report.. B. The carrier shall:. 1) Submit the request to the Executive Director of the Commission not later than 15 days before the survey report is due;2) Explain the reason for the extension request; and. 3) Propose a reasonable date certain for submission of the survey report.. C. For good cause shown, the Executive Director shall grant a reasonable request for extension..
