09.08.03.01.htm 09.08.03.01. 01 Scope.. This chapter shall apply to all claims arising from transactions or events occurring on or after July 1, 1988.

A claimant who seeks compensation from the Home Improvement Guaranty Fund (Fund" shall file a claim with the Commission on a claim form prepared by the Commission. This claim form shall contain at a minimum:1) The amount claimed based on actual loss;. 2) The facts and circumstances giving rise to the claim; and. 3) An affirmation by the claimant that all statements in the claim are true..
