An operator shall submit to the office, before expiration of the initial license:1) A request for a continuing license, on a form supplied by the office;. 2) Signed and notarized permission to examine records of abuse and neglect of children and adults for information about all individuals, as applicable, specified at Regulation .02B(1) of this chapter; and3) Any other documentation required by law or regulation.. |
